About: MICHAEL FORDE, DDS, MD Chief clinical Officer, Paradigm Oral Health Dr. Forde received his DDS from the University of Southern California and his MS in Biomedical Sciences and certificate in Prosthodontics and Maxillofacial Prosthetics from the Mayo Clinic. Dr. Forde owned a private practice limited to prosthodontics in Roseville, CA from 2008-2016. Dr. Forde was the owner of the Sacramento and Elk Grove ClearChoice Dental Implant Centers from 2015-2024. Dr. Forde was the Vice President of Clinical Affairs and later the Vice President of New Center Development for ClearChoice Management Services between 2021 and 2024. During his time in private practice and at ClearChoice, Dr. Forde restored approximately 5,000 arches of fixed full arch restorations and has leveraged that experience in helping to evolve the technique from an analog to a digital workflow. Dr. Forde is a Fellow of the American Academy of Maxillofacial Prosthetics, a member of the Academy of Osseointegration, the Mayo Clinic Alumni Association, the California Dental Association and the American Dental Association.
