Calcium channel blockers (CCBs), specifically nondihydropyridines like diltiazem and verapamil, have pronounced effects on chronotropy at therapeutic doses. Overdoses can results in life-threatening hypotension and shock from bradycardia, loss of inotropy, and vasodilatation. They are, therefore, a leading cause of non-ischemic cardiogenic shock and poisoning mortality. No high-quality randomized controlled trials are available to present clear recommendations on the efficacy of therapeutic interventions. However, available data does provide some direction and various organizations have provided guidelines. This presentation reviews the latest available information and recommendations on medical management of acute CCB overdose, as well as indications for initiation of extracorporeal support (VA ECMO). |

Despite advances in clinical and systems management of patients with ACS over the years, mortality for patients with ACS and cardiogenic shock, as well as non-ACS presentations of cardiogenic shock, has not changed significantly. Recently, efforts have been initiated around the world to improve the care of this patient population from first contact in the field to multidisciplinary cardiogenic shock teams in the ICU. This presentation reviews evidenced-based early recognition of cardiogenic shock in the prehospital and critical care transport environments.
