Meeting Summary:

Eastern Oregon Coordinated Care Organization (EOCCO) invites you to join us at a community meeting to discuss the future of healthcare under the State of Oregon’s next phase of coordinated care, “CCO 2.0.” 

This meeting presents an opportunity to highlight community-driven health improvement efforts in your community, and to consider approaches we can take to improve the system together. 


Presentation Topics:

  1. How EOCCO will engage community members and healthcare providers in improving the health of the community and addressing regional, cultural, socioeconomic, and racial disparities that exist among members and the community. 
  2. How EOCCO’s approach will meet the Triple Aim of better health, better care and lower costs for the population it serves.
  3. How EOCCO’s approach will address the following areas:
  • Value-Based Payments
  • Health Information Technology 
  • Social Determinants of Health & Health Equity
  • Behavioral Health
