Severe Weather and A New Chief…. The not so perfect storm.?

On September 17, 2019 severe weather threatened Chesterfield County Virginia, resulting in several incident responses from a EF2 Tornado. The tornado covered approximately nine miles of destruction path, caused serve damage to several large structures, and was responsible for several building collapses along the Hull Street Road corridor. This class will focus on the lessons learned from the after-action report, to include organizational, incident and timeline overview, incident response, mitigation, demobilization phases. We will then take a detailed look at the lessons learned from the above areas so that other may learn from our experience. The class will include audio of first-hand video footage, incident wave file, and other auditory components to illustrate the lessons reinforced and learned. Those lessons include command presence, ICS organizational and strategic guidance during high risk, low frequency emergency events. I look forward to a engaging in a facilitated discussion during our time together.

Time is the enemy in cardiac arrest. Once the heart stops, rescuers often can’t physically reach the victim quickly enough and/or act effectively to save them. We have reached an impenetrable wall called time. We need to cheat time. This presentation will reveal how the apparent lack of benefit from standard ACLS treatments -- like defibrillation, epinephrine, antiarrhythmic drugs -- is due more to our delay in their administration than their lack of efficacy. This presentation will discuss the need to reconfigure how we respond to life-threatening emergencies like cardiac arrest, supply and empower lay bystanders to initiate definitive treatments earlier while EMS units are enroute, and employ novel strategies allowing the human body to withstand longer periods of resuscitation with full neurological recovery.

Launched in October 2015 by the White House, Stop the Bleed is a national awareness campaign and a call to action. Stop the Bleed is intended to cultivate grassroots efforts that encourage healthcare professionals and the lay public to become trained, equipped and empowered to assist in a bleeding emergency before professional help arrives. No matter how rapid the arrival of professional responders, bystanders will always be first on the scene. A person who is bleeding can die from blood loss within five minutes, therefore it is important to quickly stop the blood loss. Those nearest to someone with life threatening injuries are best positioned to provide first care. According to a recent National Academies of Science study, trauma is the leading cause of death for Americans under age 46. Remember to be aware of your surroundings and move yourself and the injured person to safety, if necessary.

You will learn what everyone should know to stop bleeding after an injury. This course focuses on treatment in the first few minutes after the injury. Once you have successfully completed this program, we will teach you how to become a Stop the Bleed instructor so that you can take this information to people in the areas you serve. Stop the Bleed can be taught in schools, businesses, and churches.  A great outreach training opportunity for your EMS agency.

In this physically demanding program the students will learn to treat victims of structural collapse, crush/impalement injuries, and patients in limited access situations such as industrial areas. The trauma modalities covered will include crush syndrome, hypothermia, dehydration, major hemorrhage, dust impaction, and many more. Students will train at the Henrico Fire structural collapse range in very tight spaces. They will crawl through pipes, operate in tight pancaked structures, and over rubble to assess, treat, package, and extricate victims. Equipment needs and communication difficulties will also be covered at length.  This is a class for physically fit and able providers of both fire, EMS, and emergency room staff (RN, MD, PA). You will be expected to be willing and able to crawl through very tight and dark spaces, move heavy mannequins, and treat patients in very uncomfortable environments. Launched in October 2015 by the White House, Stop the Bleed is a national awareness campaign and a call to action. Stop the Bleed is intended to cultivate grassroots efforts that encourage healthcare professionals and the lay public to become trained, equipped and empowered to assist in a bleeding emergency before professional help arrives. No matter how rapid the arrival of professional responders, bystanders will always be first on the scene. A person who is bleeding can die from blood loss within five minutes, therefore it is important to quickly stop the blood loss. Those nearest to someone with life threatening injuries are best positioned to provide first care. According to a recent National Academies of Science study, trauma is the leading cause of death for Americans under age 46. Remember to be aware of your surroundings and move yourself and the injured person to safety, if necessary.

You will learn what everyone should know to stop bleeding after an injury. This course focuses on treatment in the first few minutes after the injury. Once you have successfully completed this program, we will teach you how to become a Stop the Bleed instructor so that you can take this information to people in the areas you serve. Stop the Bleed can be taught in schools, businesses, and churches.  A great outreach training opportunity for your EMS agency.

On May 31st, the unthinkable occurred in Virginia Beach. An employee entered a City building opening fire on colleagues, killing 12 and severely wounding 4. The attack triggered a massive law enforcement, EMS and fire response.  Rescue task forces deployed as police officers addressed the active shooter threat.  While the immediate crisis was resolved relatively quickly, a significant recovery period followed.   EMS personnel supported numerous activities including memorials, member welfare and continuity of operations. This presentation will review both the response and recovery efforts from an EMS/Fire agency leadership perspective.

The AIRVO 2 generates high flows of warmed and humidified respiratory gases, delivered to spontaneously breathing patients through a variety of nasal, tracheostomy and mask interfaces., An innovative oxygen system, providing and effective, versatile solution for the delivery of oxygen therapy for both pediatric and adult patients. The flow may be from 2 – 60L/min depending on the type of interface.

As a result of prospective, randomized clinical trials, another option has emerged for the patient with hypoxemic respiratory failure. Heated, humidified, high-flow nasal cannula oxygen has been available for over a decade, but refinements and increasing clinical experience have made it a solid alternative for management that exists in the spectrum of options before noninvasive and invasive mechanical ventilation. This modality was initially developed for neonatal patients, and refinements have permitted its use in adults. Conventional oxygen therapy is not well tolerated at high flow rates because of problems with unheated and non-humidified oxygen. The high-flow nasal cannula oxygen systems are able to heat and humidity, improving patient tolerance and comfort.
